Every week, Divine Home Care dedicates 8 hours of free, professional care services to community organizations and individuals who need support but face financial barriers. Healthcare access should be available to everyone.

How to Access or Partner
For Individuals & Families
Eligibility: You must demonstrate financial need and have a care-related need that aligns with our services.
Application Process: Complete a brief application form sharing your situation and care needs.
Next Steps: We'll contact you within 3–5 business days to discuss your needs and schedule services if approved.
For Organizations
Partnership: We seek organizations serving vulnerable or underserved populations.
How It Works: We discuss your organization's needs and create a partnership plan.
Next Steps: Together we identify populations served and schedule regular support.

Frequently Asked Questions
We provide personal support services including bathing assistance, meal preparation, light housekeeping, companionship, mobility assistance, and medication reminders. We do not provide medical/nursing care through this program.
Service hours vary based on need and availability. During the intake process, we'll discuss what level of support is appropriate for your situation.
No. There is no cost to apply or receive services through the Community Initiative. All services are completely free.
Most applications are reviewed within 3-5 business days. Once approved, we typically schedule services to begin within 1-2 weeks.
Yes! Community organizations can refer eligible individuals. We welcome referrals from social workers, community centers, and nonprofits.
We regularly assess your needs and adjust services accordingly. If circumstances change, simply contact us to discuss adjustments.
